Please call the office to make an appointment in advance.
If you are unable to keep your appointment, please call at least 24 hours in advance so the team may use that time to see another patient in need of care.
The office makes a sincere effort to adhere to the appointment schedule and appreciates your patience if delayed due to unforeseen circumstances.
If you are a new patient, please arrive to your appointment 15 minutes early in order to complete the registration process.
The office makes every effort to decrease the cost of medical care.
Co-pays are collected at the time of service. Cash, checks, MasterCard, Visa, and Discover are accepted for convenience.
Please request all prescriptions and authorizations during regular office hours, when full records are available.
